A 55-year-old, male patient has been recently diagnosed with diabetes mellitus and placed on glimepiride (Amaryl) titrated to 8mg daily. He also has hypertension, a history of alcoholism, and a cigarette smoking habit. He is 5 feet, 9 inches tall and weighs 144 pounds. About four weeks ago he developed extreme tiredness and dry mouth with excessive urination. He went to the doctor’s office where his blood sugar was 400+ mg/dl on two occasions. He was advised to be admitted to the hospital where his blood sugars could be brought under control. While in the hospital, he was placed on insulin and a 1200 calorie diet. The patient and his wife received education about nutrition. He also saw an ophthalmologist because of cloudy vision. He is a heavy smoker and attempts to get him to quit the habit were futile. On chest x-ray he shows a flattened diaphragm. EKG was normal. He was complaining of feeling hungry and insisted on going home. Since his blood sugars were going down and he was feeling less lethargic, he was discharged with the following medications: Catapres (clonidine), 0.2 mg, BID, NPH insulin (20 units daily in the morning), and a 1500 calorie diet.
