rephrase. pc from PCG stating medications was not at the pharmacy that was called in last nigte. PCg also reported that she doesn’t feel mom pain medication is working. Sw inquired if pt was in excreting pain no and need a nurse visit today. Pcg reported SN visit could wait until tomorrow she just was worried about her mom eyes. Sw spoke with Patient care Manager and she reported that medication was called in and that awaiting medical director signature. PCM sent message to medical director. Sw updated pcg. Sw also informed pcg to talk with SN during visit tomorrow about pain managment and eye drops. PCg stated she would and ask specifically if SN would give oxy for pain. Sw informed PCG that Skilled nursing will assess pt for pain and prescribe medications as needed. SW also notified SN to ensure pain medication is manage closely due to there being several people in the home as well as PCG. PCg reported that she meet with a man name Lanford and he offered to buy the house and the family pay him monthly. SW advised PCG to do her research to ensure that the information is ccurate.
