You are making client rounds on the night shift on a very busy surgical unit.

One of your clients, Mr. Burton, is still awake at 0230 hours. His surgeon, Dr. Cole has booked him for an orchiectomy for stage 2 testicular cancer. The surgery is to be done at 0800 hours in the morning.

You comment on his not being able to sleep. He says cannot sleep as he is worried about his surgery. Before offering him prn sedation, you sit at his bedside and ask him if he wishes to talk. During discussion, he asks you to go over what is going to happen as he wants to be clear about what he was told in the “Preparing for Your Surgery and After . . .” class held a few weeks ago.

You review his understanding of the preoperative preparation with him and you ask him what he recalls about his diagnosis, treatment and post-surgical follow-up. His explanation of his surgery is similar to that of a vasectomy and it is clear that he does not understand the surgical procedure or outcome of his scheduled surgery. Mr. Burton then requests that you give him his prn sedative so he can try to sleep
