The students will form 2 groups. Group 1 will assume Client 1-3. Group 2 will assume Clients 4-6. Each group will review and gather information on their clients and create plan of care/ plan for discharge. The team will then hold a care conference for their clients with each student role playing a different Multidisciplinary Team Member.Â

Acute Care Client #1
Jim Clarke is a 76 years old client who was transferred to the ward last night. Last evening Mr. Clarke was not able to control his angina pain so he called 911. The ambulance took him to the ER where he was diagnosed with a Myocardial Infarction. He was diagnosed with Atrial Fibrillation He was given ASA in Emergency and started on IV Heparin.
Mr. Clarke has a history of type II diabetes, angina and hypertension. Mr. Clarke lives alone but had support from his daughter Grace and son-in-law Paul. Grace and Paul assist Mr. Clarke with his home maintenance, grocery shopping and some light cooking.

Abigail Sampson is a 25 year old client who is admitted to the ward 2 days ago. Abigail’s admitting diagnosis is a closed head injury and fractured left radius resulting from a workplace accident. Abigail is employed at a local department store; while stocking shelves she fell 3 feet from a ladder and hit her head on the concrete floor. Abigail has been nauseated since the accident and is experiencing transient confusion. Abigail’s boyfriend Pedro is her only support in town, aside from a few friends who all work full time.

Tamika Rudd is a 40 years old high school teacher who has been experiencing fever, lower abdominal pain, nausea, vomiting and diarrhea for the past 3 days. Tamika presented to the ER yesterday and was admitted to your surgical ward last night. Diagnostic tests are all pending to determine the cause of Tamika’s symptoms. The admitting Physician’s diagnosis is “query appendicitis” but Tamika is awaiting a surgical consult to determine if the diagnosis is correct. Tamika is quite worried about her son James who is 13 years old. Tamika is a single mom and is trying to find a place for James to stay while she is hospitalized.

Alexander Jones is a 82 years old client on the surgical ward. Mr. Jones was diagnosed with BPH a few years ago and has been managing the symptoms at home until recently. Mr. Jones was admitted pre-operatively for a trans-urethral prostate resection that will be performed in the next few days. Mr. Jones has no other remarkable medical history. Mr. Jones and his wife, Lola who is 81 years old, live alone and are fully independent. They both drive and care for their property and each other. Lola has expressed concern about maintaining their home on her own while Alexander is hospitalized and during his recovery period at home.

Aro Dihnn is a 65 years old retired lawyer who has been admitted to your ward following a left sided cerebral vascular accident. Mr. Dihnn was diagnosed 5 days ago and has been transferred to your ward from the Intensive Care Unit where he was stabilized. Mr. Dihnn has a history of alcohol dependence and hypertension. Mr. Dihnn’s wife of 40 years has not left his side since he was brought to hospital. Mr. Dihnn is on swallowing precautions and is going to require support for all his care and ADLs.

Benita Mendez is 79 year old woman who lives at one of the local extended care facilities in your community. Mrs. Mendez has a history of dementia, type II diabetes and osteoporosis. Early this morning, Mrs. Mendez was found on the ground beside her bed by one of the HCA’s in the extended care facility. While there was no obvious injury noted by staff, Mrs. Mendez was transferred to the ER for assessment as she was complaining of right hip pain. Diagnostic tests revealed a fractured right hip and within an hour of the fall a large hematoma on the back of her head was present. Mrs. Mendez has no family in town but her son plans to fly in from Toronto to visit her. The surgeon has put Mrs. Mendez on the OR slate for this afternoon to stabilize her hip fracture, he is not sure yet what measures he will need to take to accomplish this.
