Case Study: Coronary Artery Disease
Suggested Uses: Medical-Surgical
Kristin Elrod, who is 58 years old and postmenopausal, is seen
at the women’s health clinic for a routine checkup. The nurse’s
assessment identifies that Kristin is at high risk for coronary
artery disease (CAD), and the HCP schedules Kristin for
screening lab work.
The nurse responds when Kristin expresses a lack of desire to learn about lifestyle modifications. After lab results are obtained, the nurse explains the cholesterol levels and provides teaching when lovastatin is prescribed.
Several months later, Kristin is diagnosed with stable angina and nitroglycerine is prescribed. The nurse teaches Kristin about side effects and how to administer the SL tablets. Teaching about a stress echocardiogram is also done.
After Kristin undergoes surgery, she comes to the ER with crushing substernal pain. Priority care is implemented by the nurse. An acute MI is diagnosed and the nurse notifies the HCP about the client’s recent surgery. The nurse recognizes associated ECG and cardiac isoenzyme changes, and assesses Kristin’s symptoms. IV nitroglycerine is administered and the client is transferred to MICU, where the nurse takes action when ventricular tachycardia develops.
PTCA via femoral insertion is performed and the nurse provides care after the procedure. When Kristin is transferred to Progressive Care, the nurse prioritizes the care of four assigned clients, including Kristin. In time, the effectiveness of discharge teaching is evaluated and Kristin is discharged
