LTG: The client will maintain clear airways and prevent future episodes of ineffective clearance by 2300,05/14 (4 days). STG: The client will demonstrate improved tolerance to activity evidenced by the decrease in reported SOB during mild exertion by 1700, 105/15 (within five days). LTG: The client will demonstrate a measurable increase in tolerance to activity with the absence of SOB, fatigue, and vital signs within the normal range by 1200,05/24(two weeks). This was my goal but instructer told me to correct.the following text are from instructer he short-term goal is too long. Next quarter, remember the line will be a very short line for you to write on. How can you shorten this goal and make it measurable? How about something like the client stating his cough has reduced by 50%? With the long-term goal of days or weeks, it is way too much time. Also, we talked about the long-term goal being only 24 to 48 hours. The reason we do that is because if the goal is too far off, then we realize days later or weeks later that the patient is not moving towards that goal. Short increments of time help the patient reach those goals. In addition, the long-term goal is not measurable – what one person thinks is a clear airway the next person may not. It is also likely not realistic that a COPD patients will consistently have clear airways. In addition, a goal that we are preventing future episodes of COPD exacerbation is not realistic. We can only take care of the issue at hand now, not issues in the future.
